Mission: Crusader Community Health serves the Rock River Valley area with quality primary health care for all people in need.
Location: Only CHC in Northwest Illinois; 2 locations in Rockford (West State & Broadway); Belvidere; Crusader Community Health Loves Park opened September 2011.
Funding: $30.7M budget (only 18% federal grant assisted funds); 285 employees (67 healthcare providers – MD, DO, DDS, PA, NP, CNM; All physicians are board certified or board eligible); no volunteer providers are utilized.

- School based preventive dental exams started during 2011, with follow-up restorative dental care at Crusader
