Since 1972, Crusader Community Health has strived to ensure that all persons in our community have access to quality primary health care.

Crusader Community Health plays a vital health care role in the community, serving patients unable to afford the rising cost of health care.  While there are no restrictions on who can utilize its services, Crusader specializes in serving medically underserved individuals.  Without Crusader, a broad and growing segment of our population would fail to receive the primary health care that reduces the cost of catastrophic illness.

The community's demand for affordable healthcare through Crusader is growing.  On an annual basis, over 42,000 individuals receive care from Crusader Community Health.  This accounts for over 150,000 individual patients visits. 

Nearly 97% of Crusader Community Health patients have incomes at or below 200% of the federal poverty level.  Approximately 35% have no health insurance and another 50% are Medicaid recipients.

Today, Crusader Community Health is the only community health center located in northwest Illinois.  Services are provided on an affordable, sliding discount basis and integrate health promotion and disease prevention into a program of comprehensive health care.

Crusader Community Health is governed by a community board of directors.  The Crusader Health Foundation is a not-for-profit corporation organized to support the growth and development of Crusader Community Health.
