Crusader Community Health's Health Care for the Homeless Program was established in 1988.  Its mission is to provide comprehensive medical care for homeless individuals within the Rockford area.  The program consists of physicians, dentists, nurse practitioners, nurses and case managers committed to helping clients access medical services.  Services are provided within Crusader Community Health and at a variety of sites throughout the community.

The picture to the right was taken by a client of Crusader Community Health's Health Care for the Homeless Program as a part of the "Day in the Life" photo exhibit.  In 2005, to promote awareness of the problems of homelessness in the community, Crusader Community Health gave cameras to 70 staff and patients and asked them to take pictures of what is meaninful to them.  This thought-provoking photo exhibit, which was featured at the 2005 Health Care for the Homeless National Conference, is available for display.
