Mission

  • Crusader was founded in 1972 with the mission to provide access to primary health care to people in need. Most of our patients continue care with us for their lifetime. Nine of ten (90%) of our patients come to Crusader after being referred by a friend or family member. Eighty percent of our patients tell us we exceed their expectations!
  • Crusader provides care with dignity, compassion and an awareness of cultural needs.
  • Crusader provides health care for all ages, from birth (with over 1000 delivers a year) to childhood (with over 18,000 children) to adulthood (with over 25,000 adults).  We care for 1 in 5 area residents and offer you comprehensive care.  Last year our system had provided 177,000 visits.

  • In addition to the Rockford locations, children’s dental care is available at the Belvidere clinic and the Stephenson County Health Department.
  • You can choose which site to receive care, as long as there is an opening for a new patient.

Doctors

  • All 'healthcare providers' at Crusader are American trained and board certified, which means they have passed very difficult national tests.  No volunteers are used here.
  • Our staff includes physicians, dentists, Certified Physician Assistants, Advanced Practice Nurse Practitioners, Certified Nurse Midwives and Doctors of Pharmacology.  Most work full time only at Crusader Community Health.
  • During your health care visit, you will be asked to choose a Primary Care Provider (PCP) for your medical and dental needs.  You will not rotate sites or providers, unless your personal provider is not available.
  • Our focus is out-patient (out of hospital) ‘primary care’ that involves adult internal medicine, family practice, pediatrics and women’s health.  Some specialty services are on-site; however most specialty care is handled by a referral outside our system.  We also focus on care to prevent illnesses, care to find problems early (such as cancer) and care to prevent complications from diseases.
  • Crusader providers will utilize “Best Practice” modern standards of care at all times to manage and improve your health care.

Cost

  • Crusader accepts Illinois Health Connect, Medicaid, Medicare and many private insurance plans.
  • Crusader can provide assistance in completing applications for Medicaid or other benefits.
  • Payment assistance discounts are available based on your ability to pay, family size and income.  To be considered for a discount you simply provide proof of income.  Discount levels are 25%, 50%, 75% and 100% on eligible services, and are re-evaluated annually. 
  • For those receiving a payment assistance discount, there is a minimum $20 fee due for each medical visit and a minimum $30 fee due for each dental, optical or podiatry (foot care) visit, regardless of discount.  This cannot be waived, however we will see you if you have an urgent problem or have been recently hospitalized.
  • Some services are not eligible for a discount, such as eyewear and dental procedures involving dentures, partials and root canals.
  • Crusader discounts are only available for services in our system. They do not apply to referrals outside our system, emergency room care nor hospital admissions.  You can contact the hospital or specialist to discuss receiving a possible discount or to set up a payment program for your hospital care.
  • Most laboratory and radiology tests ordered and processed at our facility are eligible for discounts for patients without insurance.

Pharmacy

  • A pharmacy is located at both our Broadway and West State Street locations in Rockford, where you can receive high quality medicine at a low cost.  Our goal is to help you afford your prescribed medication.  At the pharmacy, a Medication Assistant Resource Specialist can help you apply for medication subsidy programs offered through pharmaceutical companies.
  • If you need a refill, you can contact the Crusader pharmacies at – West State Street, 815-490-1630 or Broadway, 815-490-1350 and they will contact your provider. Think ahead and allow 48 hours to fill your medicine. The pharmacies are open 9 a.m. to 5 p.m. Monday-Friday.  If you would like your refills transferred to another pharmacy, contact that pharmacy to make the connection.
  • Prescriptions are electronically sent to the pharmacy of your choice.  There are no refills of narcotic/opiate medication after hours or on weekends by the on-call doctor.
  • Medicare Part D plans are accepted.
  • Bring all your medicine (in the original container) to every visit.
  • Narcotic and opiate medication is handled via a signed Prescription Agreement and our Pain Management Clinic. We do not tolerate prescription abuse or drug seeking behavior.

Dental Care

  • Our dental program has been recognized as one of the best in the nation.
  • Our dental equipment is state of the art, including an electronic dental record, modern digital x-rays and a flat screen TV in front of every dental chair to display films about your procedure and dental health.
  • We focus on preventive exams (in children, starting at age one or the first tooth). We also provide fillings, cleanings and emergency treatment for children and adults.

Adult Comprehensive Dental Care

  • Comprehensive adult dental care (those in need of an exam, treatment plan and follow-up care) will be available to the following populations:

    Patients receiving care through the Health Care for the Homeless Program; through the Living with HIV Program; receiving prenatal services and those with a chronic medical condition relating to cardiovascular, cancer or diabetes and referred by your provider.

  • Emergency dental care is available for adult patients on a walk-in basis. 

Hospitalizations

  • Crusader physicians and midwives are on staff at SwedishAmerican Hospital (SAH)
  • Adult and children who need hospital admission are cared for by the SAH ‘Hospitalist’ physician, after referral by the Crusader doctor/provider.  After discharge, your Crusader primary care provider (PCP) will continue your care at our facility.
  • Our outstanding Women’s Health Service staff will deliver your baby at SAH.  Certified Nurse Midwives are available for non-complicated daytime deliveries, with back up deliveries by our obstetricians.  Family Medicine doctors also deliver babies.  Crusader’s prenatal (pregnancy) program includes a Maternal Fetal Medicine (MFM) specialist to help manage complicated pregnancies. Our obstetricians also perform C-Sections and do gynecological surgery. Crusader obstetrical providers will provide your primary care at SAH.
  • Crusader does not perform Emergency Care.  If you go to the Emergency Room, tell the ER staff that you see a Crusader provider. You will be referred back to us, along with your records.

  • Crusader has over 250 employees working to help you.  All are friendly, professional and well trained.  If you need any help, just ask.
  • Many staff speak Spanish. Translation is available for all languages.

Modern Facilities and Equipment

  • Crusader works hard to provide you with a clean, attractive and modern facility. Recent renovations are focused on improvements to make your visit even better.
  • Our equipment is first rate. We have high tech electronic medical and dental records, modern dental equipment and a top of the line women’s health ultrasound.

Education

  • Crusader has a Patient Learning & Resources Center at our West State Street facility.  Go there to learn about your health concerns and illnesses.
  • Dieticians and health educators are available.
  • Pre-GED classes are on-site and taught by Rock Valley College staff.
  • Crusader offers ‘Baby Basics’ parenting classes.
  • If you are worried about an abusive relationship, you can receive private help via our specially trained staff. Please let your provider or nurse know if you are worried about an abusive relationship.
