Crusader Patients Can Now Access Care Using
Staying sheltered and practicing good social distancing are priorities during the COVID-19 outbreak. However, many of you might find yourself, or a loved one, needing the help of a healthcare professional for a variety of non-emergency needs.
There is no need to suffer, have unanswered questions or avoid seeking proper care until the COVID-19 outbreak is over.
For many Crusader patients, a TELEHEALTH (telephone) APPOINTMENT might be the perfect solution. Telehealth appointments for some types of medical care are now available to Crusader patients who have established relationships with Crusader primary medical care providers.
It’s simple! Call the main Crusader number at 815.490.1600 to schedule an appointment as you normally would. Your medical provider team may decide to contact you before your appointment if they think it can be converted to a telephone call appointment.
Appointments can be made Monday through Friday, between 8:30 am and 5:00 pm. Until the COVID-19 outbreak is resolved, Crusader has temporarily discontinued evening and Saturday appointments.
Be assured, if you need to be seen in person, Crusader medical providers will make arrangements and keep your safety and well-being top priorities. For those coming to any Crusader facility at the present time, we ask that you come ALONE. If you need assistance, you may bring ONE person.
Remember, your Crusader care provider may be able to assist you with your care while you stay in the comfort and safety of your own home.
As always, THANK YOU for entrusting your care to Crusader Community Health.
