As of 2:00 pm Saturday, April 16, 2016, phone service to Crusader Community Health has been restored.

We are sorry for the inconvenience this caused our patients. Please use this number to make appointments at any of our five locations in Rockford, Loves Park or Belvidere.

Crusader Community Health is experiencing a technical issue with its telecommunications provider. As a result the main Crusader phone number, 815-490-1600 is not currently operational. For medical emergencies please call 911. For other medical needs and to make an appointment please call 815-490-1601. This is a temporary interruption and we are sorry for any inconveniences.
