Crusader Community Health (CCH) now has the capability to use technology to share your medical records with your providers outside of CCH, communicate with you as a patient, and send appointment reminders. Please see below for information on how we are utilizing technology to better serve you.
What is an HIE? Electronic Health Information Exchange
An electronic health information exchange (HIE) allows doctors, nurses, pharmacists, other health care providers to appropriately access and securely share a patient’s vital medical information electronically — improving the speed, quality, safety, and cost of patient care.
Patient data that will be shared with your providers outside of CCH through the HIE can include the following:
- Past medical history
- Current medications
- Laboratory orders and results
- Up to date progress notes
- Patient referrals
- Discharge summaries
Only providers who participate in the Carequality or Commonwell Alliance HIE will have access to your CCH medical records if needed for continuity of care beginning February 6, 2023.
If you would like to “opt out” meaning your medical records will not be electronically sent or received from and to CCH, please come into a CCH location to complete an opt out form.
Patient Portal
My Crusader Portal provides a convenient way to access your health information online. You can view your visit summary, view lab results and medications, request appointments and communicate with medical staff. If you’d like to sign up for Crusader Patient Portal, please visit our website at www.crusaderhealth.org for information.
Appointment Reminders
CCH is now texting appointment reminders to patients scheduled to see a provider. Please reply “yes” to confirm your appointment or “no” to cancel. You can also opt out by texting STOP. This helps us manage our schedules by knowing whether you will be attending your appointment at the scheduled date and time.
