Founded in 1972, Crusader Community Health is one of 1,200 Community Health Centers (CHC) in the United States. Nationally, CHCs offer over 7,000 sites and serve almost 22 million patients.
Crusader Community Health’s mission is to serve the Rock River Valley area with quality primary health care for all people in need. Crusader serves over 57,000 patients which is about 20% of our area’s population.
Crusader is the only CHC in Northwest Illinois. We currently have eight locations: four in Rockford, IL (West State, Broadway, Alpine Road and Auburn High School); one in Belvidere, IL; one in Loves Park, IL; and two in South Beloit, IL.
Crusader is financially sound operating on a $42+ budget (only 16% federal grant assisted funds). We have over 325 employees (75 healthcare providers – MD, DO, DDS, PA, NP, CNM; All physicians are board certified or board eligible). No volunteer providers are utilized.
Crusader underwent an unannounced on-site survey in September 2017. A team of Joint Commission expert surveyors evaluated Crusader for compliance with standards of care specific to the needs of patients, including infection prevention and control, leadership and medication management. Crusader has received accreditation seven times since 1999.
