Crusader Health Education Class
The Division of Clinical Quality (DCQ) ensures clinical excellence for patients of Crusader. DCQ offers intensive case management services for patients living with chronic diseases including diabetes, asthma, congestive heart failure (CHF), and obesity (weight management). DCQ offers education services in both a classroom setting and one-to-one sessions in both English and Spanish. The clinical staff of the program includes a nurse who is a certified diabetes educator, a registered dietitian, a respiratory therapist, and a clinical pharmacist (to provide medication management services).
In addition, DCQ offers community programs throughout the area (primarily Winnebago, Boone and Stephenson Counties). Community services include blood glucose screenings for diabetes through the Cosmopolitan/Crusader Community Health Diabetes Program and public speaking engagements on a variety of health related topics.
