PAYMENT ASSISTANCE

Any patient can apply and may qualify for a reduced fee based on income and household/family size.

There are four discount levels available on eligible services:

25%, 50%, 75% and 100%

Eligibility for discounts is evaluated annually or as income and household/family size change.

Regardless of discount level, the following minimum fees are due at the time of service:

Behavioral Health
(mental health & substance use)  

                                                     $15 per visit

Medical, Telepsychiatry           $25 per visit

Specialty, Dental                       $35 per visit

TELEHEALTH

Ask your provider about connecting virtually through a telehealth visit. In some cases, a patient and provider can meet virtually using a smart phone, tablet or computer instead of meeting in-person. Together, the patient and medical team will decide what technology works best. They may also decide an in-person visit is needed.
