Behavioral Health Consultant

The Behavioral Health Consultant (BHC) is co-located on the clinical unit and collaborates as part of the health care team providing treatment to patients with behavioral health needs (mental health, substance use, behaviors impacting overall health and functioning) utilizing an integrated care model. Behavioral health services are provided to children, adolescents, and adult patients and their families.

Pediatric Nurse Practitioner

The Pediatric Nurse Practitioner provides primary care services to children and/or adults as part of the provider team at Crusader Community Health collaborating with licensed MD/DO physicians.

Clinical Medical Assistant

This position performs various duties and routine technical patient care tasks in accordance with policies and procedures, and within the scope of education of a Medical Assistant.

Dentist

To provide general dental services to children and adults as part of the provider team at Crusader Community Health.

Clinical Chair of Pediatrics

The Clinical Chair of Pediatrics is the lead clinician providing primary care services to children as part of the provider team at Crusader Community Health.

Family Practitioner

The Family Practitioner provides primary care services to children and adults as part of the provider team at Crusader Community Health.

Physician Assistant

The Physician Assistant gives primary care services to children and/or adults as part of the provider team at Crusader Community Health collaborating with licensed MD/DO physicians.

Nurse Practitioner

The Nurse Practitioner gives primary care services to children and/or adults as part of the provider team at Crusader Community Health collaborating with licensed MD/DO physicians.

Obstetrics and Gynecology Physician

The Obstetrics and Gynecology physician provides obstetric and gynecological services to women as part of the provider team at Crusader Community Health.

Behavioral Health Case Manager

The Behavioral Health Case Manager works with patients (child, adolescent, adult) with behavioral health needs, including mental health, substance use, and psychosocial issues.

Clinical Documentation Improvement Specialist

Educates providers and serves as the subject matter expert for any topic related to provider documentation in the organization. Is an advisor to providers regarding complete and accurate outpatient documentation and functions as a liaison between providers and coders with the goal of creating mutual understanding of the many uses of medical record documentation.

Women’s Health Nurse Practitioner

The Women’s Health Nurse Practitioner provides primary care services to children and/or adults as part of the provider team at Crusader Community Health collaborating with licensed MD/DO physicians.

Systems Engineer

The Systems Engineer plans, analyzes, installs, and supports the clinics’ central computer systems and network including servers, operating systems, and software. This position participates in the planning of infrastructure to support new applications and supports Crusader employees with their critical technology needs.

Dental Assistant

To provide patient care and assist providers under the immediate direction of the Dental Unit Manager or designee. This position provides direct patient care in the Dental Department.

Registered Nurse

The Registered Professional Nurse (RN) coordinates nursing and medical interventions, patient care planning, education, promotes optimal patient health, and directs the care provided by the Medical Assistant and Certified Nursing Assistants within the scope of the State of Illinois Nurse Practice Act and in accordance with the Code of Ethics for Nurses.

Prenatal Care Case Manager

This is a direct service position in the Women’s Health Services Department that provides services to Crusader prenatal patients.

Medical Unit Manager

The Medical Unit Manager is a registered nurse who provides support to the patient care teams under the direction of the Director of Clinical Operations.

Health Educator

The Health Educator is a member of a team of professionals that support the work of the medical providers by educating patients regarding health promotion, disease prevention, and management of chronic diseases.

Certified Medical Coder

The Certified Medical Coder is responsible for timely, accurate and comprehensive abstraction of provider services from the medical record, utilizing appropriate CPT-4 procedure and ICD-10 diagnosis codes.

Clinical Informatic Medical Assistant

The Clinical Informatic Medical Assistant is responsible for maintaining an expert level of knowledge on managed care organization’s medication prior authorization processes to promote medication adherence.

Dental Customer Care Representative

The Dental Customer Care Representative is responsible for a variety of patient centered customer care activities.

Director of Clinical Operations

The Director of Clinical Operations provides operational leadership for clinical and business functions within the organization’s clinics
