Accountable Care Organization Care Coordinator

POSITION DESCRIPTION

The Accountable Care Organization (ACO) Care Coordinator supports the implementation of the patient’s integrated care plan and works towards resolution of patient needs/barriers. The care coordinator carries out delegated functions related to disease states and other conditions that have been deemed to be part of the scope of practice for unlicensed staff members. Their focus is to offer individualized assistance to patients, as well as their families and caregivers to overcome health care system and community barriers and facilitate consistent and timely medical care across the continuum of care. Care coordinators function as members of the integrated care delivery team and provide services in both the hospital setting as well as in Crusader clinics. This position reports to the Chair of the Behavioral Health Department.

EDUCATION REQUIREMENTS

  • Bachelor’s degree from an accredited college/university in the human services field (i.e. psychology, social work, sociology, nursing, healthcare, human development and family services)
  • Current or ability to obtain CPR certification.
  • Current with any required Continuing Education requirements
  • Certified Medical Assistant certification preferred
  • Valid driver’s license and insurance

SKILLS & EXPERIENCE

  • Locating, coordinating, and accessing appropriate services and entitlements for patients.
  • Case Management principles of practice including the formulation of treatment plans, ability to
  • monitor progress, and ability to modify plans to meet patient needs.
