EDUCATION REQUIREMENTS

Graduate of United States accredited Dental School DDS or DMD degree, new graduates must pass licensing exam
- Unrestricted Illinois Dental license, Illinois Controlled Substance License, Federal DEA, Illinois DDS or DMD license
- Current CPR Certification (responsible for maintaining certification)
